Abstract

Chronic obstructive pulmonary disease (COPD) is characterized by a chronic inflammatory state and extensive extracellular matrix (ECM) remodeling. The initiation and perpetuation of this inflammatory state is not fully understood. ECM damage associated molecular patterns (DAMPs) might contribute to this gap of knowledge as the extensive remodeling could lead to an increase in ECM DAMP release. Therefore, a literature study toward the ECM DAMPs biglycan, decorin, versican, aggrecan, hyaluronan, heparan sulfate, fibronectin, fibrinogen and tenascin-C is performed. ECM DAMPs are able to stimulate various pattern recognition receptors and subsequently activate inflammatory pathways e.g. MAPK, NF-ΚB, IRF3 and NLRP3 resulting in both pro-inflammatory and anti-inflammatory effects. The disturbed protease-antiprotease balance, the upregulated expression of most ECM DAMPs and increased expression of ECM DAMPs contributing to an altered lung composition imply a role for ECM DAMPs in COPD. This can be supported by several studies that show that the levels of ECM DAMPs are increased both in serum as well as in extracellular lung fluid. However, further research is necessary to make a well supported claim about the potency of ECM DAMPS as future therapeutic target for COPD.
