Abstract

Background: Severe mental illness (SMI) is a long-term psychological disorder that requires long-term care. Bipolar disorder and schizophrenia are common diagnosis that are part of SMI. Psychotropic drugs are common treatment for both disorders. Methods: Data was collected from the IMPROVE database. The study population was selected with the following criteria 1) patients were in care for longer than 2 years, 2) they once had a GAF-score lower than 50 during their trajectory, 3) patients are between 18-65 years old. Costs were calculated by multiplying the amount of DDDs by the price in the year of interest. Covariates gender, year of distribution and age were investigated. Results: From the total SMI population 11,6% had a schizophrenia diagnosis (n=3255) and 4,5% had a bipolar diagnosis (n=1255). In schizophrenia antipsychotics were most prescribed, in bipolar disorder lithium salts. The total costs between 2012-2017 in SMI patients with schizophrenia were €3.8 million, with a mean annual cost per patient of €1068,90 ± 3411,98 (median €336,38). The total costs of SMI patients with bipolar disorder between 2012-2017 were €660 thousand with a mean annual costs per patient of €616,23 ± 1216,80 (median €166,16). Conclusion: The obtained insight into the costs and utilization of psychotropic drugs in SMI patients could help improve guidelines for the treatment of both disorders, which might decrease treatment costs for these patients
