Abstract

Every year the National Institute for Public Health and the Environment (RIVM) collects a large amount of data regarding the antibiotic use of nursing homes in the Netherlands. This is called the surveillance netwerk infectie verpleeghuizen (SNIV). This network is created to monitor the infections that occur in nursing homes every year and to alarm the nursing homes for potential outbreaks. The problem with the current situation is that every year less nursing homes tend to participate to the voluntary SNIV protocol. The main goal of this study was to research which results could be obtained with the collected data of SNIV 2016, what the collected data means and how the SNIV protocol can be improved so eventually more nursing homes will participate. The results showed that the Amoxicillin Clavulanic acid was the most prescribed drug in the nursing homes and the urinary tract infection is the most common infection in the nursing homes. Azithromycin has the highest prescribed dose in one nursing home and the average usage in nursing homes in April was 5.81% with 8 nursing homes passing this average and in November 6.46% with 9 nursing homes passing this average. There were two main reasons from all the nursing homes to not to participate to the current SNIV protocol. The nursing home did not have enough time to carry out the registration or they did not have enough personnel to participate, meaning they were to busy. Analyzing this problem, the advice to offer help to the nursing homes with limited time was introduced, meaning that RIVM personnel can go to the nursing home and carry out the registration for them. This saves time and resources for the nursing home and the RIVM can collect more data than before. This advice was found to be so good that the RIVM decided to implement it immediately, causing more nursing homes to participate. In the future, the system can be improved by digitalizing the whole process, meaning that the nursing homes do not have to do the registration anymore but the RIVM can log into their electronically client’s dossier (ECD) and collect the needed information without having to go to the nursing home or the nursing homes having to make time.
