Abstract

INTRODUCTION: Medication adherence in type 2 diabetes mellitus (T2DM) is associated with better health outcomes and less healthcare utilisation. The economic impact and impact on healthcare utilisation of medication adherence to T2DM treatment has been reviewed. However new medicine groups have been introduced. AIM: This structured review aims to examine the effect of medication adherence to glucose lowering agents on healthcare utilisation and costs in T2DM patients. METHODS: A literature search was performed in MEDLINE incepting to August 2024 to identify studies focusing on the association between medication adherence and total or pharmacy costs, or inpatient, outpatient and ER healthcare utilisation. Studies were screened by two reviewers independently and data was extracted and summarised descriptively. RESULTS: Of 1,239 studies identified, 36 studies have been included in this review. 26 studies reported on the association between adherence and costs;16 reported that higher medication adherence to T2DM treatment was significantly associated with a decrease in total healthcare costs ranging from $-110 till $-11.124 per patient per year. Significantly higher pharmacy costs were found in 15 out of 22 studies ranging from $444 till $5.982 per patient per year.
